Application to your health insurer

Fill in the application and the cost estimate here. At the end you download both as one finished PDF, sign it and send it to your insurer.

The document is in German because that is what your insurer needs.

How it works

  1. 01

    A certificate from your doctor

    Ask your GP for a certificate of medical necessity (Notwendigkeitsbescheinigung) or a prescription. The form is below.

  2. 02

    Fill in the application and the estimate

    You do that here, in four short steps. The finished PDF arrives straight away.

  3. 03

    Send it all to your insurer

    The application and estimate, your doctor’s certificate and my proof of qualification. Most insurers only decide before the first session.

  4. 04

    Book the appointment once they agree

    As soon as the approval arrives we arrange the appointment. If they refuse, write to me anyway.
